چکیده

With recent advances in neuroimaging and neurosurgical techniques as well as better understanding of functional motor brain circuits, there is increasing evidence that electrical deep brain stimulation (DBS) can be a powerful and safe therapeutic option for medically intractable movement disorders and neuropsychiatric diseases. For more than half a century, the ventrolateral (VL) nucleus of the thalamus has been an anatomical target for stereotactic surgeries (e.g., ablation surgery and DBS) treating various types of tremors, including Parkinson’s disease (PD)-associated tremor. The VL nucleus comprises the nucleus ventralis intermedius (Vim) and the nucleus ventralis oralis (Vo) (Hassler, 1959). Currently, thalamic DBS is widely used because of its reversibility, adjustability, and low level of invasiveness. Thalamic DBS of the Vim nucleus can effectively alleviate parkinsonian tremor; however, it does not provide consistent improvement in parkinsonian symptoms other than tremor. Therefore, its use is limited to patients with tremorpredominant PD, in whom other motor symptoms are not disabling. In this review, we will survey the rationale and current use of thalamic DBS in the treatment of PD.

Relationship of the P Angle with Stereotactic Arc in Intraoperative Outcome of Patients with Parkinson’s Disease Undergoing Deep Brain Stimulation

Background & Aim: DBS (deep brain stimulation) is a new and successful technique in treatment of symptoms of Parkinsonism especially after drug resistance. Research in this field is mostly designed for evolution of this technique. The present study aimed at evaluating the relationship between the angle formed in midsagittal and STN (sub-thalamic nucleus) axis line and recording l...

From parkinsonian thalamic activity to suppression by Deep Brain Stimulation: new insights from computational modeling

This paper presents a computational model of a thalamocortical (TC) relay neuron that is used to explore the behaviour of the basal-ganglia thalamocortical loop in relation to Parkinson’s disease and deep brain stimulation. Low frequency oscillations associated with Parkinson’s disease impair thalamic processing by generating rebound action potentials. In the model the thalamocortical cell rece...

Deep Brain Stimulation: a review of the literature for established, new and experimental clinical applications and targets

Introduction Deep brain stimulation (DBS) is an established surgical treatment for Parkinson’s disease (PD), essential tremor and dystonia. It is generally acknowledged that the development of DBS as we know it today started with the publication of Benabid, Pollak et al in 1987 on thalamic DBS for tremor. However stereotactic surgery for PD was very common in the pre-levodopa era, and the vast ...

Critical Review: What is the effect of subthalamic deep brain stimulation on swallowing in persons with Parkinson’s disease?

This critical review examines the effects of subthalamic nucleus (STN) deep brain stimulation (DBS) on swallowing in persons with Parkinson’s disease (PD) in 8 studies. Study designs include: within group, pre/post-test studies and case reports. The evidence from this review indicates that STN DBS did provide positive outcomes for swallowing function in persons with PD; however a large degree o...
